LinCU Application Requirements

  • Completed LinCU Application form. Click here to access the LinCU Application Form.
  • Two (2) forms of Valid Identification (i.e. National Identification Card, Driver’s Permit, Passport)
  • Proof of Address must carry applicant’s name (i.e. Utility Bill – must be within 3 months.)
    N.B. If the utility bill is not on the applicant’s name, written consent and valid identification are required from the bill owner to use the bill
  • Proof of Employment / Income (Job Letter – within 3 mths. /Payslip – within 1 mth. or recent Pension Statement)

Please note the signature on the LinCU application form must be identical or similar to that on the identification submitted by the member.

Job letter ( no older than three months) or pay slip (no older than one month). Weekly paid employees require the last four payslips while fortnightly paid employees require the last two payslips.
Utility bills must be no older than three months (WASA - no older than six months). If the bill is not on the applicant's name a signed and dated authorization letter and valid identification is required from the name of the person whose name the bill is on.
